Implemented in #1309.
The form is covered by a unit test suite that checks the rendering, every validation rule (an empty form, a token amount that is not a whole number, a token amount above the maximum, a max price of zero and terms that were not accepted) and the submission (a market order, a bid order and the removal of the error summary). The accessibility suite was extended with the name and the description of the form, the required fields, the error summary and the focus management.
The PR also covers #1268, because the tests belong with the change they cover.
