A fix for another file that is not applied (eg. the header is not analyzed) is recorded on the diagnostic's note. The thread comment, the PR review's suggestions and the review summary now say which file was left out and why, so users can apply that part themselves.

with skipped fixes is not approved, and suppress the “No concerns to report”

RestClient::post_feedback and the no-concerns condition in
ReviewComments::summarize, preserving existing behavior when the list is empty.

A review with skipped cross-file fix parts asks the user to apply them, so it should not approve the PR or say there are no concerns.
Only the list items that fit within GitHub's 65535 character limit (with room for the outreach text) are added; the number left out is noted.

Resolves #490
Before: when part of a clang-tidy fix changes another file and #501 could not apply it (eg. the header is not analyzed, is ignored by
--ignore-tidy, the change is outside its changed lines, or it overlaps another fix), the only trace was an info-level log line. In the thread comment and the PR review, the diagnostic looked fully fixed, so the header declaration could silently stay out of sync with its definition. After: the thread comment, the PR review and the review summary say which file was left out and why, eg.
How:
TidyNotificationgetsskipped_fixes: Vec<SkippedFix { file, reason }>.apply_other_file_fixes()records each rejected fix on the note about its diagnostic in the analyzed source file (once per file and reason).make_tidy_comment()adds a:warning:line under the diagnostic.TidyAdvice::get_suggestion_help()adds it under the diagnostic in the suggestion, and so does the comment for diagnostics without an applied fix.ReviewCommentscollects them inskipped_fixes, andsummarize()lists them, so they are also shown when suggestions do not fit in the diff orCPP_LINTER_PR_REVIEW_SUMMARY_ONLYis set.docs/pr-review-caveats.mdexplains when a fix for another file is applied.Summary by CodeRabbit
