Deny camera access because the application only requests microphone access.

camera=(self) allows same-origin code to request camera access. The voice-note flow requests audio only. Change the policy and its test assertion to camera=().

Review comment at @app/Http/Middleware/SecurityHeaders.php at line 28:
Update the PERMISSIONS policy in SecurityHeaders to deny camera access by
setting camera to empty while keeping microphone access enabled; update the
corresponding Permissions-Policy test assertion to match.

CWE: CWE-209 — Generation of Error Message Containing Sensitive Information

Disable debug mode before the production guard throws.

If production runs with app.debug enabled, the guard throws a LogicException during boot(). Laravel's HTTP kernel catches boot exceptions and passes them to the exception handler. The handler renders detailed output while app.debug remains true, so every request returns a stack trace, not only requests that hit an application error. Set app.debug to false before the exception is thrown. The handler then renders a generic error page, and deployment still fails. Check that no custom exception rendering in bootstrap/app.php overrides this path.

Review comment at @app/Providers/AppServiceProvider.php around lines 48 - 49:
Update the production debug guard in AppServiceProvider so it captures whether
production debug mode is enabled, sets app.debug to false before throwing, and
uses the captured condition for the guard.

Prepend SecurityHeaders so early responses receive the security headers.

Laravel 13 places appended middleware after PreventRequestsDuringMaintenance. Maintenance responses can therefore skip SecurityHeaders, omitting HSTS on HTTPS and the other headers. Add header assertions to the maintenance test.

Apply the middleware ordering and test the early response

Review comment at @bootstrap/app.php at line 46:
Update the middleware registration for SecurityHeaders to prepend it so
maintenance responses receive security headers. In the maintenance-response test
in ErrorPageTest, assert the expected security headers are present, including
X-Content-Type-Options, Referrer-Policy, and Permissions-Policy.

Keep handle-based admin access off when ADMIN_GITHUB_IDS is set but parses to no IDs.

Security Misconfiguration

Reachability: External
Exploitability: Difficult
CWE: CWE-636

If ADMINS=admin remains set and ADMIN_GITHUB_IDS=0, array_filter() removes "0" because it has no callback and PHP treats "0" as falsy. The same happens with ADMIN_GITHUB_IDS=,. The filtered list is empty, so User::isAdmin() falls back to handle matching. An account that holds the released admin handle can then pass UpdateFeatureRequest authorization.

Enable the handle fallback only when ADMIN_GITHUB_IDS is blank. A nonblank value that yields no IDs must deny admin access. Use a separate flag for "configured" and leave the ID list unchanged.

Add a configured flag in config/app.php
     'admin_github_ids' => array_values(array_filter(array_map(
         trim(...),
         explode(',', (string) env('ADMIN_GITHUB_IDS', '')),
     ))),
+
+    'admin_github_ids_configured' => trim((string) env('ADMIN_GITHUB_IDS', '')) !== '',
Check the configured flag in User::isAdmin()
-        if ($ids !== []) {
+        if (config('app.admin_github_ids_configured')) {
             return in_array((string) $this->github_id, array_map(strval(...), $ids), true);
         }

Review comment at @config/app.php around lines 62 - 65:
In the admin GitHub ID configuration, add a separate flag indicating whether
ADMIN_GITHUB_IDS is nonblank, leaving admin_github_ids parsing unchanged. Update
User::isAdmin() to use that flag to decide whether handle-based fallback is
allowed, so a configured value that parses to no IDs denies admin access.
