Summary

Add a utility skill that looks back at agent run-log issues and produces ranked, actionable suggestions for improving skills and process.

Changes

  • New skill skills/utility/retrospective/ (SKILL.md + agents/openai.yaml).
  • Documented in pipeline-guide, skills/utility/README.md, and root README.md.

How it works

  • Lists agent:log issues (label list, not search).
  • Reads entries (YAML header preferred), clusters Problems / Things to improve, ranks suggestions by frequency and impact.
  • Report-only: does not edit labels, issues, or skills unless the caller explicitly asks after the report.

Evidence / verification

  • Skill scaffold matches the utility pattern (merge-safety style Inputs / Process / Report / Never).
  • Guide table and utility README list the skill.

ADR check

No conflict with existing ADRs. After merge, maintainers may want npm run sync so plugin metadata and runner prompts pick up the new skill if required by the packaging pipeline.

Closes #37.
