Summary

A triage pass that fails to set labels can leave the outcome only in a comment. A later pass with label capability was not reconciling because it only looked at the latest comment / current labels.

Changes

  • Instruct triage-github-issue to read every comment on the issue before setting state labels.
  • If a prior trusted Agent Brief / Triage Notes / ready-for-human structure already recommends a state, reconcile the label to match (unless newer evidence supersedes it).
  • Require **Runner:** $runner on every outcome (and other) comment this skill posts.

Evidence / verification

  • Skill process steps renumbered: new step 7 (read all comments + reconcile), outcome post is step 8, set state is step 9, release is step 10.
  • Inputs table documents that runner is recorded in comments as well as the run log.
  • Never list updated to forbid setting state without reading all comments.

ADR check

No conflict with existing ADRs.

Closes #35.

Before setting the state label, read every comment on the issue (not only
the latest). If a prior trusted outcome comment already recommends a state,
reconcile the label to match. Record the runner name on every outcome
comment the agent posts.

Closes #35.
