"github-agent-workflow": minor
---

Add a third Reviewing outcome, handed to a person (ADR 0008). A PR that runs out of `max_review_rounds` no longer goes to `agent:stuck`: it gets the new label `agent:human-merge` and a comment that @mentions its CODEOWNERS (read from the base branch) and says what is still open. The new input `human_merge_paths` (gitignore-style patterns, empty by default) names files an agent must never merge: once such a PR passes review it is handed to a person the same way. `agent:human-merge` is ignored by Shepherding and Reviewing, holds no merge lane, and restarts `review_rounds`, `shepherd_attempts` and `failures`. Triaging's housekeeping now also removes `agent:pr-open` when it closes an issue whose PR was merged. Run the label setup in `references/labels.md` again to create `agent:human-merge`.

# 0008. Hand a reviewed PR to a person to merge

## Status

Accepted

## Context

Reviewing could end in two ways: merge, or send back. Two cases fit neither.

1. A PR runs out of `max_review_rounds` and still is not mergeable. The only exit was an escalation to `agent:stuck`, which says an agent gave up and something is broken. In practice the PR is usually reviewable and a person only has to decide. A `stuck` label does not say that, and it named at most one `human_reviewer`.
2. Some changes should never be merged unattended however well they review. In a repository where agents improve the agents' own instructions, the merge gate, the trust rules, the scripts and the release workflow are such files. ADR 0007's CI cannot tell whether a prose change alters agent behavior, so a green check is not enough.

## Decision

Add a third outcome, **handed to a person**, with its own label `agent:human-merge` and `references/human-merge.md`.

- **Two triggers.** `review-rounds`: the rounds ran out and the PR still is not mergeable. `protected-paths`: review passed, but the PR changes a file that matches the new input `human_merge_paths`. For a protected path the agents still review, fix and re-review as usual, and only the final merge is left to a person. They do not hand over unreviewed code.
- **The comment is honest.** It @mentions the owners and says why an agent is not merging, what was checked at which SHA, what is still open, and what to do. It never calls a PR safe beyond what `merge-safety` and `code-review` reported.
- **Owners.** CODEOWNERS is read from the PR's **base** branch, never its head, which the PR could edit. The owners of the changed files get an @mention and a review request, together with `human_reviewer`. With no CODEOWNERS file, `human_reviewer` is the only target and the PR is still labelled and commented.
- **Fail closed.** `protected_files` fails when the changed files cannot be read or are empty, or when a pattern is unsupported, and Reviewing then fails the item instead of merging. A pattern that silently matched nothing would let a protected file through.
- **Path patterns.** A deliberate gitignore and CODEOWNERS subset, shared by both uses (`*`, `**`, `?`, a leading or trailing `/`), in `helpers.mjs`. No negation, character classes or escapes. A renamed file counts under its old name too, so a file cannot leave a protected path by being renamed.
- **Ignored by the agents.** `agent:human-merge` is excluded like `agent:stuck`, and it holds no merge lane (ADR 0007).
- **A person's decision restarts the counts.** The `agent:human-merge` marker resets `review_rounds`, `shepherd_attempts` and `failures`, as `agent:escalate` already resets failures. A PR a person hands back with `agent:needs-work` is not escalated again on its first review.
- **No shipped default.** `human_merge_paths` is empty. Which files need a person is the repository's policy, passed in by the runner like any other input.

## Consequences

- `agent:stuck` regains its meaning: something an agent could not do. `agent:human-merge` means a decision is waiting for a person.
- A person merging a PR that is behind its base must update the branch first when the base requires it. The comment says so.
- When a person merges, no agent runs the merge path that closes the linked issue and removes `agent:pr-open`. Triaging's housekeeping now does both for any issue whose PR is merged.
- This is a policy gate, not a security boundary. It stops an agent following these skills from merging a protected path, but a token that can merge can still bypass the skills. Branch protection or a ruleset on the base branch that requires a code-owner review is what enforces it.

# Hand a PR to a person to merge

Use when an agent reviewed a PR and should not merge it itself, but the PR is not broken: a person should read it, and merge it if they agree. Only Reviewing calls this. It is not an escalation. `escalate.md` is for an item an agent could not make progress on.

## When

- **`review-rounds`.** `review_rounds N` has reached `max_review_rounds` and the PR still is not mergeable. Open findings remain, or `merge-safety` or `code-review` still will not pass.
- **`protected-paths`.** `merge-safety`, `code-review` and the PR-format check all passed, but the PR changes a file that matches `human_merge_paths`. The agent has done its review and a person decides.

A fork PR is not this: follow `escalate.md`.

## Do

1. **Find the owners.** `OWNERS=$(codeowners_for N)`. It reads CODEOWNERS from the PR's base branch and prints the owners of the changed files, one per line, or nothing if there is no file or no owner. Add `human_reviewer` if it is set. If `codeowners_for` fails, carry on with `human_reviewer` alone and note it under **Problems** as `degraded`.
2. **Post one comment** on the PR. @mention the owners first. Then, in this order and plainly:
- why an agent is not merging it, in one sentence: the matching files for `protected-paths`, or the rounds used and what is still open for `review-rounds`;
- what the agent checked at `$SHA`: whether checks are green, the `merge-safety` verdict, the `code-review` result;
- for `review-rounds`, each finding that is still open. Do not write "safe to merge" when `merge-safety` said BLOCK or HOLD: say what it said;
- what to do: review it, and merge it if you agree; update the branch first if it is behind its base; to send it back to the agents, push your fixes or leave comments, remove `agent:human-merge` and add `agent:needs-work`, which restarts the review-round count.

End it with `<!-- agent:human-merge reason=<review-rounds|protected-paths> sha=$SHA -->`.
3. **Relabel**, in one command: remove the PR's state label (`agent:ready-for-review`, or `agent:needs-work` if it has that) and add `agent:human-merge`.
4. **Ask for review** from the owners and `human_reviewer`, leaving out the PR's author and any email address: `gh pr edit N --add-reviewer \"<user>,<org/team>\"`. This is best effort. A person without access cannot be requested, and the @mention has already notified them. Note a failure under **Problems** as `degraded` and carry on.
5. For a PR with a linked issue, post a one-line comment on the issue: `PR #N is waiting for a person to review and merge it.` Leave `agent:pr-open` as it is.
6. **Release your lease** (`release N pr agent:in-progress`).
7. **Log it** (`log.md`) with the result `#N handed to a human: <reason>`. Add a **Problems** entry tagged `escalated` only for `review-rounds`. `protected-paths` is the policy working, not a problem.

`agent:human-merge` makes Shepherding and Reviewing ignore the PR. It holds no merge lane. Only a person removes it. The marker restarts `review_rounds`, `shepherd_attempts` and `failures`, so a PR a person has taken over and handed back is tried afresh.

## Never

- Merge the PR, approve it, or dismiss anyone's review.
- Call it "safe" or "ready to merge" beyond what `merge-safety` and `code-review` actually reported.
- Read CODEOWNERS from the PR's head.
