Fixes #18554. Original report: #9607 (closed in the V2 sweep, still reproducible on Orchestrator V2; see this comment). This rebuilds #11048 on ClaudeAdapterV2.

Problem

T3 keeps one Claude CLI process alive per thread. When that process is holding an OAuth session that expires and can't be refreshed, every turn in the thread fails with Failed to authenticate: OAuth session expired and could not be refreshed. Running claude auth login again doesn't help: the next message goes to the same process, which keeps failing until the idle reaper kills it or the user presses Stop. The error text also tells the user to start a new thread.

openQuery reuses the live process whenever the native thread, policy and model selection match, and nothing records that the process failed to authenticate.

Fix

  • When a turn ends failed after the CLI reported authentication_failed, the adapter marks that live process as signed out. The next turn's openQuery skips reuse and takes the existing replace path: it closes the process and opens a fresh one that resumes the same native thread, so it picks up the current login.
  • Other failures, and interrupted turns, keep reusing the process.
  • The sign-in error says "then send the message again" instead of "then start a new thread".

This is a small, focused fix for an obvious bug: one flag on the live query, one reuse condition, and the error text.

Complementary to #15915, which replaces a session when the provider instance is rebuilt (API key or base URL changed in Settings). A login from a terminal doesn't rebuild the instance, so that change doesn't cover this case; this change stays inside ClaudeAdapterV2.openQuery and doesn't touch ProviderSessionManager.

Before / After

Isolated dev state, a throwaway project, and an isolated CLAUDE_CONFIG_DIR whose OAuth session is expired with a dead refresh token. The thread fails, then claude auth login succeeds for that config dir, then the same message is sent again in the same thread.

Signed out After claude auth login, same thread
Before (main 5fe7568)
After (this branch)

Verification

  • New test in ClaudeAdapterV2.test.ts, parametrized over the failure kind:
    • authentication_failed: the next turn closes the old process and opens a new one with resume set to the same native session.
    • server_error: the next turn reuses the same process.
    • Both wait on the turn-terminal event; no sleeps or polling.
  • Same test with only ClaudeAdapterV2.ts reverted to main:
× after a turn fails with authentication_failed, the next turn opens a fresh process: true
AssertionError: expected 1 to equal 2
    Tests 1 failed | 1 passed | 231 skipped (233)

  • vp test run apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts apps/server/src/provider/Drivers/ClaudeHome.test.ts: 2 files, 238 tests passed.
  • tsc-rs --noEmit in apps/server: no errors. vp lint on the touched files: no new findings. vp fmt --check: clean.
  • Manual, Claude CLI 2.1.296, web client against isolated worktree state (screenshots above): on main the same thread still fails after a successful re-login; on this branch the next message succeeds and shows the new error text.
  • Not covered by the fix: a process that started with no credentials, or one whose login was removed with claude auth logout, already recovers after login on main, so those paths behave the same before and after. Desktop runs the same server code; mobile has no server-side change.

Model and harness: Claude Opus 5.5 via Claude Code in T3 Code.

🤖 Generated with Claude Code

The Claude CLI reads its login once per process, and a thread keeps one
process alive between turns. After a turn fails to authenticate, mark
that process signed out so the next turn replaces it with a fresh one
that resumes the same conversation. The sign-in error tells the user to
send the message again instead of starting a new thread.

Approvability

Verdict: Not approved

Macroscope's review found this PR not approvable — This focused fix changes production behavior around Claude authentication-session recovery by replacing a failed CLI process and resuming the existing conversation. Because authentication handling is sensitive, the change warrants human review despite its limited scope and targeted tests.

After a Claude authentication failure, the adapter marks the live query signed out and opens a new process for the next turn. The new process resumes the existing native session. Subscription-login guidance now tells users to send the message again.

The adapter marks a live query signed out after an authentication failure and does not reuse that process for the next turn. Tests verify that a fresh process resumes the native session, while a server error leaves the existing process in use.

Merge Risk: 🟡 Moderate · up to 3456c

A result-only authentication failure could leave the same thread using an expired process after login. Handle that result before merging unless this remaining recovery gap is accepted.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 3456c

Recovery preserves the existing conversation and access settings. No introduced privilege expansion was identified, but ownership guarantees during overlapping recovery attempts are not fully established.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • observed — The changed recovery branch acts on the adapter-owned live process for the existing native conversation. Replacement receives the same application thread and provider-session identifiers, configured instance environment and workspace inputs; the change adds no authority-bearing input.

Trust Boundaries and Controls

  • observed — Replacement uses the existing runtime-policy derivation, MCP tool configuration, permission mode and approval callbacks. Incoming frames from a no-longer-current query are rejected, and stream-exit cleanup checks query-object ownership before clearing or finalizing replacement state. These controls predate the PR and remain in place.

Resilience and Maintainability Implications

  • inferred — Ordinary queued starts have meaningful serialization and stale-attempt controls: queue progression rejects blocking runs, and startup conditionally advances the current starting attempt before reaching the provider. These reduce the concurrent-replacement concern, but do not establish complete exclusion of overlapping distinct recovery starts. The adapter's active-turn guard alone is not an atomic reservation around opening and installing a query.

Hardening Proposals

  • proposed — Establish an explicit single-owner or idempotent replacement guarantee for overlapping stale and recovery starts, including interrupted opening. This would close the remaining process-ownership proof gap; it is not an observed PR vulnerability.
