feat(decision-engine): initialize publishable package

Summary

• Add publishable @rineex/decision-engine package
• Provide a typed, immutable initDecisionEngine() API
• Configure TypeScript, Vitest, ESLint, dual CJS/ESM builds, and npm publishing metadata

Verification

• pnpm --filter @rineex/decision-engine test
• pnpm --filter @rineex/decision-engine check-types
• pnpm --filter @rineex/decision-engine build
• pnpm --filter @rineex/decision-engine lint
• npm pack --dry-run
• pnpm test (push hook)
