Major refactor: introduced BootstrapContext/Factory for service lifetimes; added PolicyService, PolicySnapshot, and RegistryPolicyReader for robust, testable policy access; telemetry pipeline now DI-driven with NullTelemetryService and explicit consent handling. Overhauled exception handling with GlobalExceptionHandling, UnhandledExceptionHandler, and user-facing crash dialogs. Logging pipeline is explicit and testable with DiagnosticMessageLoggerProvider. Single-instance logic clarified and hardened. Rewrote docs for clarity and LLM use; added documentation guidelines. Updated/added tests for new behaviors and edge cases. Minor: added ApplicationInsights dependency, improved error messages, clarified comments.
